CMS posted the updated MDS 3.0 Quality Measures User’s Manual – V12.1 on Wednesday, September 4, 2019. The 110-page manual is effective October 1, 2019. The MDS 3.0 QM User’s Manual V12.1 contains detailed specifications for the MDS 3.0 quality measures (QM) and supersedes V12.0 that was posted on February 21, 2019.
Also on September 4th, CMS posted the updated Skilled Nursing Facility Quality Reporting Program User’s Manual – V3.0 which is also effective October 1, 2019. This 106-page manual is found in the Downloads section of Skilled Nursing Facility (SNF) Quality Reporting Program Measures and Technical Information webpage along with these additional files:

The Risk Adjustment spreadsheet contains several tabs. Start with the Overview tab then move to the Schedule for Risk-Adjustment tab. The latter tab shows each Measure Calculation Date. Don’t forget to click the + symbol to the far right of the tabs in view – there’s more measures to view.
